Case Management
Case management includes an intake process, which seeks to identify client barriers through a comprehensive assessment, an identification of resources and the development of a client work plan for achieving goals identified through the assessment process.
Process towards goal attainment is measured at 7 day intervals. Facilitation of goal attainment is provided on an ongoing daily basis.
